LINKAGE COORDINATOR (Full-Time) - FORT WORTH

The Linkage Coordinator position provides linkage to medical care for persons diagnosed with HIV using the Anti-Retroviral Treatment and Access to Services (ARTAS) program model. The scope of work will include accessing and establishing a relationship with the young adult, gay and bisexual men community; young transgender persons; and other at-risk populations.
Provide linkage to care using ARTAS for clients newly diagnosed with HIV or for those living with HIV whom have been lost to care or never received care.
Establish ongoing communication and rapport with the young adult, gay and bisexual men community as well as young transgender community in Tarrant County and the surrounding counties.
Maintain fidelity of the ARTAS program.
Provide client education on the importance of HIV medical care and treatment adherence.
Link clients to medical care and follow-up, as needed.
Maintain client files as outlined in policies and procedures and program guidelines.
Maintain caseload sufficient enough to meet the goals of the program.
Maintain outcome measures for the program.
Maintain a high degree of confidentiality regarding agency matters and client records.
Participate in agency meetings, retreats, work sessions and events as requested.
Perform other related tasks as assigned.
Conduct oneself in a professional and culturally sensitive manner at all times.
Knowledge of HIV/AIDS, HIV prevention and high-risk populations and cultural knowledge and sensitivity; excellent written and verbal communication skills; working knowledge of basic computer programs and reliable transportation; knowledge of community resources.
Experience with the young adult, gay and bisexual men community; young transgender persons; and other at-risk populations.
Strong interpersonal skills, including the ability to demonstrate empathy for the purpose of providing one-on-one emotional support for clients.
Good verbal and written communication skills, well organized, and detail oriented.
Ability to work with multidisciplinary team.
Ability to cooperate with and relate well with individuals from diverse socioeconomic and cultural backgrounds.
Ability to establish and maintain linkages with service providers and programs.
Ability to work as part of a multidisciplinary team, including other Peer Navigators and healthcare providers in clinical and outreach settings.
Ability to work independently.
Ability to interact comfortably with diverse groups (ethnicity, sexual orientation, socioeconomic status, etc.)
Ability to help clients identify risk reduction strategies.
Valid driver's license and access to a car on a daily basis with appropriate insurance coverage required.
Bilingual English/Spanish preferred.
Salary, non-exempt; full-time; some evenings and weekends; some travel required; mileage reimbursed. Competitive fringe benefits and excellent paid time-off package. Formal training provided.
